Travel For Care Grant
The need to travel for fair and adequate medical care for your child is not a journey anyone anticipates, or financially plans for. We understand that in order to advocate for your child, you may be displaced, out of work and put in an unimaginable position.
We are honored to provide grants to eligible families so your child can always remain your focus.

Details:

Grants of up to $1,000 are available for eligible families

Funds may be used in the way that best serves the families traveling for care
-
Funding payments may only be made to the guardian of the child with a trisomy diagnosis

Families may submit an application for a grant at least one month apart, up to 3 times, with a maximum 3 grants totaling $2500
PRENATAL:

$250 grants are available for eligible families

Funds may be used in the way that best serves the families traveling for care
-
Funding payments may only be made to the guardian of the child with a trisomy diagnosis

Families may re-apply 6 weeks apart. With a maximum of 3 total grants, and the cut off for second and third grants being 36 weeks
Eligibility:

Family must be traveling > 50 miles

Applicant must be the guardian of the child with Trisomy 18 or Trisomy 13

Live in the U.S.
Application Checklist:

Applicant must provide Driver’s License and photo of a utility bill at current mailing address if different than address on license

Medical Documentation:
-
Provide proof of trisomy diagnosis on official letterhead signed by provider, or from medical chart, with child or guardian’s name, date of birth, diagnosis, and treating facility or diagnosing entity (lab corp) visible. For Prenatal cases only: we may accept similar documentation to support suspicion of a trisomy diagnosis, if further testing is being forgone.
